The data controller is MUDr. Pavel Holeka. For anything concerning personal data, write to us at mudrholeka@gmail.com.
The only place on this site where you enter personal data is the appointment request form. You fill in your name, phone, e-mail, location and a message. In addition, the server automatically records your IP address and the time of sending — solely to protect the form from being flooded by bots.
We use the data solely to get back to you and arrange an appointment. We do not sell it, and we do not use it for advertising or profiling.
Please do not describe your health problems in detail in the message. The form is for arranging an appointment; we will discuss your condition in person at the practice, where the rules on medical records apply.
The processing is necessary so that we can respond to your request — that is, to take steps at your request before providing healthcare (Art. 6(1)(b) GDPR). Recording the IP address rests on our legitimate interest in protecting the form from misuse (Art. 6(1)(f) GDPR).
The request is read by the doctor and the nurse of the relevant practice. Technically the data is held by:
The site uses no analytics or advertising tools and loads nothing from third-party servers — images, maps and fonts are all stored here. No other company learns anything about you while you browse.
We keep an appointment request for no more than 12 months after the request is handled. Deletion does not depend on anyone remembering: 12 months after it arrives the request disappears by itself, including the IP address and the time of sending. If you become our patient, your medical records are governed by separate legislation and kept accordingly.
You have the right to know what data we hold about you, to have it corrected or erased, to restrict its processing, to object, and to receive it in a portable form. Just write to mudrholeka@gmail.com — we will reply within a month at the latest.
If you feel we are handling your data badly, you can turn to the Czech Office for Personal Data Protection, Pplk. Sochora 27, 170 00 Prague 7.
